Why the Outback Eye Service’s Future Hangs in the Balance Despite Decades of Success

Each year the itinerant clinic evaluates between 1,200 and 1,300 patients across Bourke, Lightning Ridge, Walgett, Cobar and Brewarrina; roughly 150 of those undergo cataract or oculoplastic surgery, while an estimated 30 % of the roughly 40,000 residents in the catch‑area suffer from preventable or treatable ocular disease that would otherwise culminate in blindness.

The financial architecture is a precarious jigsaw, composed of three non‑permanent streams: annual grants from the federal Western Primary Health Network (approximately $338,000), a comparable contribution from the Fred Hollows Foundation (about $199,000), and assorted state‑level allocations—all of which are subject to yearly renewal.

Compounding the fiscal fragility is an ageing workforce; Professor Coroneo, chairman of the Department of Ophthalmology at UNSW, admits, “I’m nearer the end of my career than the start,” while Ms Barton warns, “If one of these sources pulled out tomorrow, we’d all be gone.” Their apprehension reflects a broader anxiety that the service could collapse as its senior clinicians retire.

NSW Health Minister Ryan Park has publicly pledged that the government “will never turn away additional funds” for the OES, yet Barton and Coroneo continue to press for structural certainty, stating, “We need to know we are part of NSW Health — that we’re a permanent fixture in the health system,” and “Our ask is to please embed us in the public system as an everyday service.”

Beyond restoring sight, the service averts secondary health crises—preventing falls, fractures and the attendant loss of independence that afflict remote elders; as Ms Barton observes, “We’re talking about broken hips.” Without a stable funding conduit, the region’s most vulnerable populations risk reverting to the stark alternatives of traveling over a thousand kilometres for care or, tragically, succumbing to preventable blindness.
