N.T.L., born in 2004 in Hanoi, finally achieved a successful embryo transfer at the end of 2025 after four years of waiting and a previous pregnancy loss. The hopeful start quickly turned into a high‑risk situation.

By the 12th week of gestation her cervix had shortened to 25 mm, and doctors diagnosed severe gestational diabetes. She needed up to 70 units of insulin each day to keep her blood sugar under control.

At 16 weeks a cervical cerclage was placed to support the shortened cervix. However, by the 19th week the cervix had dilated by 2 cm, raising the risk of infection and pre‑term labor.

When she reached 24 weeks the membranes ruptured prematurely, the amniotic fluid almost disappeared, and the cerclage stitch was removed. These events pushed the pregnancy toward an inevitable early delivery.

On 26 April the baby was born at exactly 24 weeks. Intensive neonatal care, including ventilation and specialized nutrition, allowed the infant to survive despite the extreme prematurity.

Dr. Nguyen Viet Thang explained that extending gestation by even a few days and maintaining strict control of gestational diabetes are crucial for improving outcomes. He advises women with threatened pre‑term labor to have regular check‑ups and to seek prompt hospital care if diabetes or other warning signs appear.

The case shows that early monitoring, timely cervical cerclage, diligent diabetes management, and the effort to prolong pregnancy can together raise the chances of survival for babies born extremely early.
