How Proper Referrals Can Cut Your Medical Bills
Quynh Anh, a 40‑year‑old from Hung Yen, discovered kidney failure and visited two major hospitals in Hanoi. Because she did not follow the facility‑level transfer rules, the Health Insurance Fund paid nothing, even though her benefit rate was between 80 percent and 100 percent, and she spent more than VND15 million. After she returned to her registered clinic, obtained an official transfer to Bach Mai Hospital and complied with the transfer regulations, her out‑of‑pocket expense dropped to only a few hundred thousand VND per visit.
The article compares two hypothetical patients with the same 80 percent benefit rate. Patient A received a proper referral from a primary‑level clinic to a specialized hospital and therefore obtained the full 80 percent coverage for all eligible expenses. Patient B went directly to the specialized hospital without a referral and was limited to a reimbursement of only 32 percent of the eligible costs.
Not every visit needs a referral. Dr Nguyen Phu Tuong, a doctor at the National Children’s Hospital, explains that emergency cases and treatments at basic‑level facilities are covered at the full benefit rate under emergency‑care regulations. Moreover, from 1 July 2026 the amended regulations broaden benefits for self‑referred patients, correcting the belief that self‑referrals always lose coverage.
Patients diagnosed with rare diseases or conditions listed in Appendix I may skip the referral step. Under Circular 01/2025/TT‑BYT they can go straight to a specialized hospital and receive 100 percent of their entitled benefits.
The key takeaway is simple: following the correct referral pathway can turn a potentially huge bill into a modest expense. Knowing when a referral is required—and when it is not—helps you use your health‑insurance benefits to their fullest.
