There was no rule about what to do if someone asked to go to the toilet.

Ms Rolla knew that Mr Grosvenor had gone missing from the hospital before while being supervised.

The court asked Ms Rolla if she thought about following Mr Grosvenor to the toilet.

Ms Rolla said she did not think it was right to follow a man to the toilet.

The hospital has changed its rules since Mr Grosvenor's death.

Julie Chapman hopes the inquest will help get justice.

Mrs Grosvenor's sister, Julie Chapman, said the system failed them.

Ms Chapman said that Mr Grosvenor had mental health issues and was violent towards his mother.

She thinks that mental health services should have done more to help.

Ms Chapman wants mental health to be taken seriously.
