Study Links Cefepime to Higher Mortality – Lessons for Clinicians

The research, published in JAMA Network Open, examined 110 randomized clinical trials that together enrolled over 22,000 patients who received either cefepime or another beta‑lactam antibiotic.

Across all trials, 778 deaths (6.6%) occurred among the 11,726 patients treated with cefepime, compared with a 6.2% death rate in patients who received a different antibiotic. Deaths were counted within about 30 days of treatment.

Using a Bayesian meta‑analysis, the researchers calculated a 94.4% probability that cefepime is associated with higher all‑cause mortality. When the analysis was limited to the 73 peer‑reviewed trials, that probability rose to 98.6%.

The association was most evident in adult participants and was strongest among those treated for febrile neutropenia, a serious condition marked by fever and very low white‑blood‑cell counts.

The investigators highlighted dosing difficulties as a possible explanation – low doses may fail to clear the infection, while high doses can cause neurotoxicity. They did not advise stopping cefepime, but called for clearer dosing guidance and further research.

For doctors and patients, the study underscores the importance of careful dose selection and ongoing monitoring when using cefepime, especially in high‑risk adult groups.
